



At this first visit, our objective to identify the long term orthodontic needs for you or your child. Our New Patient Coordinator, Nikki, will assist Dr. Roberts with your exam. She will take (with your permission), a set of photos and a panoramic x-ray of the mouth. This will give Dr. Roberts the information he needs to give you a clearer picture of your treatment options.
After your exam, one of three outcomes will be determined:
Dr. Roberts will review his findings with you and answer any questions you may have. Nikki will then show you the different options Dr. Roberts has recommended and will help formulate a treatment plan. It is our hope that you find this first visit relaxed and informative. If you find that you have some questions later, please call us. We are always available to clarify your treatment options.

When you call to schedule your first visit, we will send you a New Patient Form along with a Privacy Form and Health Insurance Portability and Accountability Act Form (commonly known as HIPAA) to fill out and bring with you to your visit.
